The 2018 Ceremony will be held on Saturday, May 19th, 1-3 pm.
- A reminder email will be sent in late winter/early spring to those that fill out (at least the first part) of this form.
- You MUST hit the SUBMIT button, at the end of the form, and you should see a screen like this:
- If you DON'T see a screen that looks similar to this,it means that you have missed a required item on the form that you have missed. Please go back through and correct any missing pieces of information.
- You should also receive a confirmation email. What you receive in the confirmation should be exactly what we receive. On this email, you will see the name of the photo you submitted, but will not be able to view your photo. If there is section for a photo is blank, it means we DID NOT receive your photo. If you don't receive the confirmation email or if you see any items missing from your submission, please let us know (firstname.lastname@example.org).
