The Ohio State Veterinary Medical Center Equine Field Services team provides your horse(s) with the highest quality care necessary to maintain optimal health and performance.

To enhance this effort, we are now offering horse owners a new option and innovative program to ensure their horses are properly cared for year round.

The new Ohio State Veterinary Medical Center Equine Wellness Program contains all the basic preventive care services we recommend, scheduled and coordinated at least twice annually by us, at a reduced cost to you.

This comprehensive package—offered in three service levels—may include:

• Physical exams with health reports
• Vaccinations
• Parasite control
• Dental care
• Coggins testing
• Call charges
• Blood work
• Soundness exam
• Sheath/udder cleaning

To get started just follow the steps below:

1. Download our enrollment form
2. Print the enrollment form and fill it out
3. Sign the enrollment form
4. Fax form to 614-688-5642, or email electronically to VTHAccounting [at] cvm [dot] osu [dot] edu.
5. Alternatively, mail the form in with a check to:
